Senior Case Manager
Full-time - Benefits eligible
Deliver structured and compassionate case management services, supervise junior staff, coordinate services, and serve as a key link between clients, community resources, and internal services at TMFC.
Accountabilities
- Conduct client intake assessments and develop goal-oriented care plans.
- Support clients with referrals, paperwork, and service access.
- Provide advanced client support including home visits and grocery delivery as approved.
- Maintain accurate case documentation and reporting.
- Supervise and mentor Junior Case Managers, delegating tasks appropriately.
- Participate in team meetings, training, and collaborative case reviews.
Education & Experience
- Bachelor’s degree in social work, Human Services, or related field; equivalent experience considered.
- 3–5 years case management experience, preferably community-based or nonprofit.
- Experience with at-risk populations and data entry.
- Spanish or Haitian Creole preferred; Microsoft 365 proficiency a plus.
