Senior Case Manager

Thomas Merton Family Center | Bridgeport, CT

Deliver structured and compassionate case management services, supervise junior staff, coordinate services, and serve as a key link between clients, community resources, and internal services at TMFC.

Accountabilities

  • Conduct client intake assessments and develop goal-oriented care plans.
  • Support clients with referrals, paperwork, and service access.
  • Provide advanced client support including home visits and grocery delivery as approved.
  • Maintain accurate case documentation and reporting.
  • Supervise and mentor Junior Case Managers, delegating tasks appropriately.
  • Participate in team meetings, training, and collaborative case reviews.

Education & Experience

  • Bachelor’s degree in social work, Human Services, or related field; equivalent experience considered.
  • 3–5 years case management experience, preferably community-based or nonprofit.
  • Experience with at-risk populations and data entry.
  • Spanish or Haitian Creole preferred; Microsoft 365 proficiency a plus.
