The qualified candidate will have:

  • strong computer and data management skills including the ability to enter, track, and interpret diverse client demographic and service data using state and federal databases;
  • HUD grants management system user certification and licensed HMIS and DDaP user is preferred. 
  • solid administrative and organizational skills and the ability to meet established deadlines. 
  • Bachelor’s Degree in administration and/or minimum two years equivalent experience working in an agency setting.

Outreach Case Manager (Full Time)

Catholic Charities of Fairfield County is seeking a dynamic individual to fill the full-time position of Outreach Case Manager with the Homeless Outreach Team in Danbury, CT.

The qualified candidate will be responsible for implementing CCFC’s strategic mission through the core function of strengths-based client support which will include, but be not limited to: street outreach, engagement, assessment, goal planning, service referral, and advocacy.  The Outreach Case Manager will be responsible for engaging with a variety of individuals experiencing street homelessness on goals such as accessing housing, benefits, clinical, medical and substance services, and other resources.

Applicants should have a Bachelor’s Degree in Human Services or related field and/or relevant work experience in the fields of mental health, substance abuse, or homelessness.   Advanced communication and problem-solving skills, valid driver’s license, vehicle, and proof of current registration and insurance, experience in data management and reporting required. Bilingual (English/Spanish) strongly preferred.

Nutrition Aide – Senior Nutrition Program (Part-time)

Catholic Charities of Fairfield County is seeking a reliable, self-motivated individual to work a flexible 8 hours/week schedule to assist in the planning and monitoring of an Elderly Nutrition Program providing both Congregate and Home-Delivered Meals.

A minimum of one year experience and/or a degree in Nutrition or Food Service Management is required. Primary responsibilities include on-site nutrition education presentations, menu planning and analysis, and site monitoring. Use of personal vehicle is required for local travel.

Catholic Charities of Fairfield County is seeking an experienced, motivated individual for a part-time Coordinated Access Network Navigator for its Homeless Services Department in Danbury, CT.

 Qualified candidates will be responsible for implementing CCFC’s strategic mission through the core function of person-centered client support which will include, but not limited to: Engaging all clients accessing the Coordinated Entry System who are not currently receiving case management supports, assessing clients’ needs related to housing in order to assist individuals and families in finding appropriate, safe, affordable housing opportunities, assisting clients with meeting basic needs, client advocacy and ensuring clients are successfully linked to resources as quickly as possible, providing referrals to and support clients in linking with both formal and informal supports.

Education:

Applicants must have a Bachelor Degree in Human Services or related field.

Experience:

At least three years of experience working with homeless individuals & families.

Skills & Requirements:

Advanced communication and problem-solving skills, valid driver’s license, reliable transportation, proof of current registration and insurance, experience in data management and reporting required.

Bi-Lingual (English/Spanish) strongly preferred
