Case Manager (Part-time)

Part-time (20 hours/week)

New Covenant Center | Stamford, CT

Provide compassionate case management for individuals and families in crisis, focusing on crisis intervention, advocacy, and connecting clients to internal and external resources.

Accountabilities

  • Conduct intake interviews and needs assessments to determine program eligibility.
  • Develop service plans to support crisis resolution and self-sufficiency.
  • Connect clients with community resources and internal programs.
  • Track client progress and maintain accurate case notes and documentation.
  • Supervise Café floor during meals and cross-train in other Operations areas.
  • Build collaborative relationships with agencies, service providers, and community partners.

Education & Experience

  • High school diploma, two-year college degree, or equivalent relevant experience.
  • 2+ years experience in case management, assessment, counseling, or crisis intervention preferred.
  • Bilingual English/Spanish strongly preferred.
  • Proficient in Office 365; organized, detail-oriented, and able to manage multiple tasks.
