Case Manager (Part-time)
Part-time (20 hours/week)
Provide compassionate case management for individuals and families in crisis, focusing on crisis intervention, advocacy, and connecting clients to internal and external resources.
Accountabilities
- Conduct intake interviews and needs assessments to determine program eligibility.
- Develop service plans to support crisis resolution and self-sufficiency.
- Connect clients with community resources and internal programs.
- Track client progress and maintain accurate case notes and documentation.
- Supervise Café floor during meals and cross-train in other Operations areas.
- Build collaborative relationships with agencies, service providers, and community partners.
Education & Experience
- High school diploma, two-year college degree, or equivalent relevant experience.
- 2+ years experience in case management, assessment, counseling, or crisis intervention preferred.
- Bilingual English/Spanish strongly preferred.
- Proficient in Office 365; organized, detail-oriented, and able to manage multiple tasks.
