Catholic Charities of Fairfield County cannot directly place people in housing; all individuals must be referred to our supportive services through 2-1-1.

Permanent Housing for Families (PHF) formally named Permanent Housing of the Diocese (PHD)
Permanent Housing for Families (PHF) is a Supportive Housing Program that provides subsidized housing and strengths-based case management for up to 7 families whose head of household has a mental health or co-occurring disorder. PHF assists families by providing subsidized housing, while focusing on economic stability and independent living through employment and education opportunities. Case Managers utilize a holistic approach to aid families in developing skills for a well-balanced life. Housing Opportunity for Persons with Aids (HOPWA)
HOPWA currently provides subsidized housing and strengths-based case management for up to 15 households. The apartments are located in the city of Bridgeport and efforts are made to ensure units match the needs of the residents pertaining to location, size, and accessibility. The program is designed for families and individuals that need financial assistance and supportive services to address their housing, mental health and/or substance abuse, and physical health needs. Successful applicants to the program have at least one household member diagnosed with HIV/AIDS, are able to confirm that their emotional health is fragile, and are chronically homeless, currently homeless, or presently living in substandard conditions. Conger Housing

Conger Program is a scattered-site, permanent, supportive housing program for 9 formerly homeless single adults living with HIV/AIDS, a specific fragile medical condition, substance abuse, or co-occurring disorder. Each applicant accepted for residency into the program is homeless, chronically homeless, or living in substandard conditions. All tenants are linked to available community resources and receive intensive case management to support their economic stability, overall good health, and well-being. Connecticut Collaborative on Re-Entry (CCR)
CCR is a supportive housing program aimed at a set of individuals in Connecticut that repeatedly cycle in and out of the homeless service and corrections systems. These individuals have complex unmet needs and histories homelessness, often combined with chronic health conditions, mental health, and substance use issues. This program provides intensive strengths-based case management. The goal of CCR is to reduce rates of re-incarceration and increase housing stability. For more information please call 211 infoline.
Merton Homes

Merton Homes is located in the Hollow neighborhood of Bridgeport, Connecticut. Merton Homes is a Permanent Supportive Housing Program (PSH). These formerly homeless families, are disabled, may have aged out of DCF services, or victims of Domestic Violence. Families are selected from the Coordinated Access Network (CAN) and are encouraged to engage in on-site intensive case management services. Merton Homes property manager and CCFC case managers work closely to provide a safe, clean, and productive environment for all 22 families. For more information please call 2-1-1 info line.

Bethlehem House
The Stratford Coalition for the Homeless, Inc., merged with Catholic Charities of Fairfield County, Inc. on October 1, 2007 to formulate Bethlehem House. The program currently operates as a project-based rapid re-housing program serving up to 9 formerly homeless families in the town of Stratford, CT. For more information please call 2-1-1 infoline.
