Case Manager
Connecticut Collaborative Reentry (CCR) Program
Catholic Charities of Fairfield County is seeking a compassionate and motivated Housing Case Manager to join our Connecticut Collaborative Reentry (CCR) housing program. This benefits-eligible position provides an opportunity to support individuals by helping them achieve housing stability, employment, and long-term success.
Key Responsibilities
- Provide strengths-based case management services.
- Complete client assessments and develop individualized service plans.
- Connect clients with community resources and referrals.
- Assist with job readiness and employment support.
- Advocate for client needs and coordinate services.
- Maintain accurate documentation, data entry, and required reports.
- Ensure compliance with supportive housing policies and federal regulations.
Qualifications
- Bachelor's degree in Human Services, Social Work, Criminal Justice, or a related field, or High School Diploma/GED with relevant experience.
- Experience working with formerly incarcerated individuals preferred.
- Knowledge of supportive housing practices and applicable federal regulations.
- Strong communication, organization, and problem-solving skills.
- Experience with data management and reporting.
- Bilingual candidates preferred.
- Valid driver's license, reliable vehicle, current registration, and insurance required.
