- From the Level 1 Dashboard select Add New PQI Scale Form.
- Enter the name of the new scale and click SUBMIT.
- On the Add PQI Scale Options Screen, add the short response (good, great…), enter the numeric value (1 as the lowest and 5 as the highest), select the PQI scale you just created. Add the qualifier text, what criteria make this worth a 1. Click ADD.
- Repeat the process for the next 4 entries in your scale.
