Community Advocacy Case Manager (part-time)

Catholic Charities of Fairfield County is seeking a part-time (18 hours/week) Community Advocacy Case Manager. Based out of our Norwalk, CT office, this position provides strengths-based case management to individuals and families experiencing an urgent financial need. The Case Manager will work in partnership with clients to address their immediate needs, offer resources, strategies and support to prevent or minimize future episodes of financial crisis.

ACCOUNTABILITIES:

  • Conduct client assessments to determine service needs, strengths, and barriers to achieving stability.
  • Provide case management services to clients, including referrals to appropriate resources, advocacy, and crisis intervention as needed.
  • Maintain accurate and up-to-date client records and documentation.
  • Collaborate with community partners to ensure clients have access to a comprehensive range of services.
  • Participate in staff meetings, trainings, and professional development opportunities.
  • Attend community meetings and events to build relationships with community partners and promote Catholic Charities services.
  • Other duties as assigned.
  • Work in partnership with CCFC programs to assist any shared clients/families.
  • Submit program data and other reports in a timely and accurate manner.
  • Travel to various locations throughout Fairfield County including soup kitchens and food pantries and other related programs.
  • Other duties as assigned.

Education and Experience:

  •  Bachelor’s Degree in Human Services or relevant case management experience with vulnerable populations.
  • Bi-lingual (Spanish/English) required.

Skills and Abilities:

  • Familiarity with community resources, Immigration issues, and SNAP benefits.
  • Advanced communication skills; oral and written.
  • Demonstrated ability to communicate in both group and individual settings.
  • Excellent interpersonal skills, comfortable collaborating with peers and external resources.
  • Ability to work autonomously and exercise flexibility as needed.
  • Advanced computer skills and strong office organizational skills
  • Valid driver’s license.
